Lead Capture Forms

Lead Forms let you build embeddable web forms that send enquiries directly into your Plenix CRM as leads.

Step 1: Create a form

1. Go to Lead Forms in the sidebar.
2. Click New Form and give it a name (e.g. "Website Contact Form").

Step 2: Build your fields

Click Add Field and choose from: Text, Textarea, Email, Phone, Dropdown, Checkbox, or Date.

For each field: set the label, tick Required if needed, add placeholder text as a hint.

Recommended fields for a contact form:

• First Name (required)
• Last Name (required)
• Email (required)
• Phone (optional)
• Company (optional)
• Message (optional)

Step 3: Configure CRM mapping

Under CRM settings, choose:

• Lead source tag (e.g. "Website", "Pricing Page")
• Assigned to — which team member receives the lead
• Pipeline stage — where to enter the lead in your CRM (e.g. "New Lead")

Step 4: Set up notifications

Add email addresses to notify on new submissions. Enable Slack notification if your connector is configured.

Step 5: Get the embed code

Click Embed on the form page.

Script embed (recommended):

Copy the script tag shown on the Embed page and paste it into your website HTML before the closing body tag.

iFrame embed:

Copy the iFrame code shown on the Embed page for use in website builders that do not allow custom scripts.

Monitor submissions

The Lead Forms dashboard shows total submissions, conversion rate, and recent submissions. Click any submission to view the full response and open the linked CRM lead.

Suggestions

• GDPR compliance: Add a consent checkbox (required). The submission timestamp and IP are stored as consent evidence automatically.
• A/B testing: Duplicate a form, change the button text or field order, and compare conversion rates after a week.
• Thank-you redirect: Set a thank-you URL so submitters land on a confirmation page on your own website.
